RELEASE OF LIABILITY, INDEMNIFICATION, AND MEDIA CONSENT AGREEMENT

This Release of Liability, Indemnification, and Media Consent Agreement ("Agreement") is made and entered into by the undersigned participant ("Participant"), and, if applicable, their parent or legal guardian, in favor of the Coffeyville Historical Society, its board members, employees, volunteers, affiliates, and representatives (collectively, the "Released Parties").

1. Acknowledgment of Risks

Participant understands and acknowledges that participation in activities associated with the Coffeyville Historical Society, including but not limited to museum tours, historical reenactments, volunteer work, fundraising events, rental events, yard and indoor cleaning work, and media rental events, involves inherent risks. These risks may include, but are not limited to, physical injury, property damage, personal liability, and exposure to outdoor elements. Participant voluntarily assumes all such risks.

2. Release of Liability and Indemnification

To the fullest extent permitted by law, Participant hereby releases, waives, and forever discharges the Released Parties from any and all claims, liabilities, damages, or causes of action, whether known or unknown, arising out of or related to participation in the activities. Participant further agrees to indemnify and hold harmless the Released Parties from any claims, damages, legal fees, or expenses incurred as a result of Participant’s actions or participation, including any damage to property that requires repair or replacement.

3. Medical Emergency Authorization

In the event of a medical emergency, Participant authorizes the Released Parties to seek medical treatment on their behalf. Participant agrees to be financially responsible for any medical expenses incurred. An emergency contact must be provided below:

4. Photography & Media Consent

Participant grants the Coffeyville Historical Society permission to use their likeness in photographs, videos, and other media for promotional purposes without compensation. This includes online, print, and other media formats.

5. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Kansas. Any disputes arising from this Agreement shall be resolved in the appropriate courts of Kansas.

6. Duration of Agreement

This Agreement remains in effect for one year from the date of signing. A new waiver must be completed each year for continued participation.

7. Severability Clause

If any provision of this Agreement is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect. The invalid provision shall be modified to the minimum extent necessary to make it enforceable while preserving the original intent.

8. Acknowledgment and Signature

By signing below, Participant acknowledges they have read and understood this Agreement, and voluntarily agree to its terms.
